Since the Saint Clair Veterans Memorial Ambulance is a Non-Profit Volunteer Ambulance Association your support is needed in keeping us running.  Operational costs increase every year.  The majority of the Organizations Income comes from the following:

Dear Friends:

Each year at this time the Saint Clair Veterans Memorial Ambulance Fund sends out a mailing to all residents in our coverage area for the membership program.  This year due to changes in Insurance Regulations we have to increase our membership.  Although this increase ,ay seem large to many of our citizens who live on fixed incomes we are still lower than many of the services in our area.  

Because of the growth in our area, we would like to review how the membership program works and it's benefits.

 1) Because the membership program is NOT an insurance policy, you may still receive an invoice from our billing office.  If we are able to obtain your insurance information before billing, the invoice will be sent directly to your insurance carrier.

 2) Typically, an insurance company will only cover a pre-determined amount of the invoice and require the patient to pay the remainder.  If you decide to enroll in the Membership program, you will not be responsible for the portion of the bill that your insurance company does not pay.

 3) The Membership program covers you for all emergency and medically necessary non-emergency transports with in the primary coverage area of the Saint Clair Veterans Memorial Ambulance Fund

 Costs:

      $35.00   Family Membership Covers Husband, Wife and all children
                   Under the age of 18.

      $25.00   Covers a single person membership.

To join the Saint Clair Veterans Memorial Ambulance Membership Fund please print out the form below and return it with your payment.  Due to Medicare and Insurance regulations, you will not be considered a member unless the minimum amount requested in received.  As always Donations in addition to the membership are greatly appreciated.  Please make your check or money order payable to:

 BILLING INQUIRES

Due to changes in Insurance regulations the Saint Clair Veterans Memorial Ambulance is using Conestoga Medical Billing in Elverson, Pennsylvania.  We feel Conestoga Medical Billing will treat our patients with the respect you deserve.  If you have a question regarding an invoice after April 1, 2002 please contact Conestoga at (610) 286-5076.
